Does anyone know how the top two tiers were achieved on this cake? I assume it's some sort of mold/impression mat but I cannot find it...have you see it???

It is beautiful.  Could very well be a homemade custom mold.

Well I wondered about that myself until I found another cake that looks to be the same mold

I've never seen anything even similar to this, but it is pretty.

My thought is they took a piece of leather and impressed it on the fondant.  Or maybe some fabric lace.....

That is a long discontinued Elegant Lace Mold by Carol Webb.  The molds are long pieces of white vacuum formed foam.
