The client wants a 2 tier cake, the bottom to be damask and have a zebra on top of the cake but I cant seem to think of what the middle tier should look like. Her colors are red, white, and black.

Maybe a placard with the baby's name or the initials. Something sophisticated and simple to play off the damask. Or the vintage ruffle effect would look nice.
