anybody ever do one? I'd like to have one this summer and invite friends and fam to sample cupcakes and fill out a short "survey". Any ideas on how to do this, how many flavor combos to have, what survey questions to ask, etc.???

I am sure you will be successful. Who wouldn't love being invited to a cupcake tasting party. I use my family as my tasters and they are honest. If you want a survey, just make it anonymous. It should be fun. I just took six new flavors to a family reunion, but I already knew they were great cupcakes. I just wanted to see how people liked the flavors. I made over 100 cupcakes for 60 people. Every single one was eaten or taken home by family members. Cupcakes being fully eaten and not in the trash is the best sign.

Suggestion... get the cupcakes cold, cut in quarters, and serve at room temp. Don't use minis because you want to make sure the real thing is good. This way guests can try all of the flavors.
