How To Do This Cake??

I have someone that wants me to do this cake. She is providing the mickey and minnie topper. Is the rest just fondant cut out and placed on top??

It does appear to be fondant cut out like a picture frame and airbrushed with a pattern. The ribbon/bow could be fondant, but it also could be done with icing. It is a cute cake.
