Cake Blowout?

Anybody guess WHAT happened here? Trapped air is the only thing that I can think of. This is a pic of what it looked like. I had already pushed it back in gently with my finger but it looked like the cake "burped."

Any ideas? Suggetions for next time?
PS: The cake was a smash cake and was not filled. Its a 6".

If the cake was frozen or cold, the icing tends to blow away from the cake surface.
