Try PM'ing some of the people who have done other "sideways" cakes...namely, some of the purse cakes. I've noticed several of these which are pretty narrow, like a book.
Is the book to be open, or closed? This could affect different techniques you might want to explore...
I am making a cake purse next week, so I have been doing a lot of looking and reading. From what I have noticed, the cake purses are short and dowels are placed from front to back at an diagonally.
A book would be taller and I am concerned that it would fall over without adequate support. I am sure there is a way because look at the cakes Duff's and others make. Of course, they use some sort of a foundation to begin with to help support the cake.

Hi! I just found this picture after posting. The thing is I just joined the website yesterday, so I hope you can use this link without having to join. It is free to join if you want to though.
The picture is a a round cake on the bottom, possibly a 12" or 14" round cake. Then on top of that is 6 books standing upright and a small round cake on top of that with a graduation cap.
I have to do a graduation cake this week-end and I have been looking at cakes with graduation caps. I hope this picture will help you.

On the book cake, it sounds like one i wouldn't dare to tackle.
