I usually use salted because that's what I normally have on hand. However, if there is also salt in the recipe and I'm using salted butter, I leave the extra salt out.

Rule of thumb, for the more modern (after '75 I think) cookbooks, if they say butter they are usually referring to unsalted. If the recipe requires salted butter they will say salted butter.
Older cookbooks usually refer to salted butter.
Another tip, it's always a good idea to search either the front of the book (if the recipe is from a book) or the back of the book of information like this. They will usually let you know if recipes were tested with salted or unsalted butter and the size eggs they used.

I go with unsalted. You can always add salt if need be.
