What Went Wrong?

I'm not new to cake baking and decorating, but problems do sometimes pop up.

I recently made a cake for my mother's birthday (lucky it was a family member's cake with an issue, lol).  I made a chocolate cake, WASC style.  The cake was 3 9in round layers, I filled it with chocolate fudge icing and frosted/decorated with SMBC.

The problem?  It was wobbly and unstable.  Layers weren't split, there were no multiple tiers.  There wasn't excessive filling.  Between the layers I made a dam of smbc around the edge of the layer before filling and adding the next layer...  but once I had it iced, it wobbled.  And then I had to drive it 10 miles to the restaurant.  It arrived intact, but had the dreaded icing bulge and in one small spot fudge icing was starting to break through.

It is still pretty! According to everything you said you did correctly, I'm not sure what could have went wrong. Maybe one our our veteran cakers can help!
