I have searched for some tips or instructions to make a head/bust cake. I have seen the ones that use styrofoam heads but I don't want to use styro., I would like to make a cake similar to this:

Her work is amazing!! I couldn't believe this was cake...wow.
Could someone please give me some tips and help or direct me on how to make the head section. could the head part be done all in cake? I don't know how to support the neck and what kind of internal structure to use to support the head?
