I use an extruder for clay that I found at the Hobby store. I can't remember the name of it off hand, but it's the green one with the twist handle. I love it, it's so handy.
I actually use a clay extruder. They are much more sturdier and won't break. It's made of metal as are the attachments. If you see the plastic ones, steer clear. They may work great at first, but they break easier since fondant is so thick. The cost of the metal clay extruders are a little high, but you won't have to buy them as often as you would the plastic ones.
I can find out the manufacturer when I get home. I'm just taking a break from work. I know you can find them online, however.
