If it technically not a baby shower cake, but baby oriented .. the sky is really the limit and you could have a lot of fun with it. You could do something really simple and pretty that just says congrats with flowers and some fun designs either piped or out of fondant OR you could get a little more fancy and make two figures sitting on top of the cake .. maybe in the grass or on a blanket, couch .. something to that effect holding a little baby. It really depends on how much of the "baby" theme you want to bring into the cake. Just had another thought .... you could also do the baby booties with a bib and the bib could say congrats ...
