I would say extract made from acohol is fine to use in royal icing. The oil part is where you have to watch it. What I would do is make the royal icing, then after it is fully made mix in the lemon extract. Once your icing is made adding a little bit of oil should not break it down. If you add the oil before you mix it the egg white won't beat up.
You can always try a small amount of icing to see how it works.
I do it all the time...haven't had any problems so far.
I use almond extract in mine ALL the time, never without. I do use Antonia74's recipe & I add mine before I add the ps
