Quilting Design Using Impression Mat

I'm trying to figure out how to get a nice, clean quilt pattern using the impression mat. The problem is that I press it onto the front of the cake, but have to press the back of the cake into the mat. Or at least hold the cake still from behind. This results in messing the back of the cake up which has already been pressed with the design.
How do you make the quilting mat work?

I use the mat as more of a guide than anything. I press the mat to the cake as hard as possible without pushing the thing off the side of the turntable. It gives me just enough of an impression to use as a guide when I go over it again with the fondant tool of choice.
I use it on buttercream. I press the middle of the mat, then the left side and almost to the right. I run my finger over the back of the mat. I don't hold onto the cake. Then I line it up on the right side and repeat. Took me a few times but works great for me.
