I am so confused about the doughshoe in these directions for the baby converse. Can someone please explain or maybe have a picture? TIA!

I have never made the shoe yet, but, I think that the dough "shoe" is just something that you can make to put the gumpaste shoe on to help shape it while it dries. I could be wrong, though. maybe someone else who has made them will know. hth
probably the best thing to do is to email or PM the person who posted the instructions.
You don't need any kind of former. I roll a snake of plastic wrap to support the toe of the shoe. I hold the sides for a few minutes until they stand on their own, then start adding the other pieces. I wad up a ball of paper towel to support the tongue of the shoe and help keep the sides in place while they dry.
The laces are just an illusion. I cut small strips and then gently tuck the ends into the holes. The bow and tails are separate pieces.
