How Do I Make A Piece Of Cake, Cake?

Hi there everyone,
Would anybody please be able to help me on making a cake that has a piece cut out of it and put on top and decorated like so many i have seen on here. I think they look awesome but im just trying to think of the best way to go about it. the cake i mean looks somthing like this:
I would probably ice and cover the whole cake in fondant, then cut out the wedge with a very sharp knife to leave a smooth line and cover the cut sides with fondant or cut the wedge first and then cover..? I would also like to hear how other people have done it...
