I have searched for the ballet slipper template but I haven't been able to find it. Some one said they got it from CakeBaker but she doesn't have pictures downloaded anymore. Can someone please help me?

Here's the link for the template I used. It was posted by cakebaker. Scroll down to the 5th post. I've made them twice now, the second time was much easier than the first.
