I have made a caramel cake covered in fondant. I was thinking that apple pie filling would be good for a filling. Is it okay to use this since it will be sitting out for a few days. I don't want anyone to get food poisoning or for it to go bad.

Is it from a can? See if the can says to refrigerate after opening. If it's from scratch, I'm not sure either; but I know my mother never refrigerated her pies. Also, you can BUY apple pies in grocery stores and they are not in the refrigerator either.

Also, most apples in the grocery store are not refrigerated-- just in a cool area, and many are left out for weeks in homes (in fruit bowls, baskets, etc.) I'm not sure, but I think it would be fine, and your cake sounds great.
