I made the lid in my pics using fondant (with gum tex) laid out over an inverted cake pan and left to dry for about 5 days. I just wrapped it like it was a cake and then trimmed the sides short to make it look like a box top. I also put a cardboard cake circle on the pan first for three reasons: 1-added stability for the finished lid 2-I was worried the fondant wouldn't otherwise release from the pan (coulda used parchment) and 3- I'd hoped the cardboard would speed the drying process since it created a layer of air between the fondant and pan. I have no idea if any of those steps were necessary, I just didn't want to take any chances. To attach it to the cake I used three dowels thru the cake (one in rear, two further towards the front...kind of in a triangle layout) sticking up to the desired height and propped the boxtop onto the cake (glued the boxtop onto the dowels w/RI or candy melts).
Every single thing I did was a result of tutorials or descriptions from people here on CC - I owe a debt of gratitute to everyone here for sharing their knowledge so that I could pull it off. You might want to search the galleries and forums for "gift box" and see what people wrote about how they did theirs...that's how I figured it all out.
Of course there is the other kind I've seen where the box "lid" is actually another layer of cake, placed on a cake board and propped w/dowels...obviously I can't help with "how to" on those kinds but I love that you get to eat the lid too. I didn't know what to do w/the fondant lid when it came time to cut the cake...I couldn't bear to throw it away, so I took it home. I still have it! LOL!
