How far in advance can you do a chocolate transfer? Can you do it a month in advance and perhaps keep it in the fridge?

Just think of it like a chocolate bar, it will last a long time provided it doesnt melt or get dust all over it. Just keep it in an airtight container. I wouldnt put it in the fridge.

Don't see why not, so long as you keep it airtight and away from moisture, but let your chocolate set up as much as it can at room temp before putting it in the fridge to finish hardening, or you may get 'blooming' on your transfer.
