Ideas On How This Cake Is Done?

A friend of mine picked out this cake from a magazine. The picture quality is poor and she has no clue what book it came from. I am thinking its a mold maybe to do the scroll work but not sure. has anyone seen this cake or have a clue maybe on how to do it?

I would do a stencil and paint it on using colored royal icing
