Order Forms...??

I have been searching for a forum on order forms that was posted some time ago. Someone posted some examples that they used for customers and I can not find it anywhere. Can someone work their CC magic and make this thread appear??

I had not seen that one yet and it is a good one. I have one that I use I will try to upload it so you can see otherwise PM me with your email and I can send it to you. Mine has lots of details, I don't know if you need one like that. Hope it helps.

This is the one that I have on my website for people to fill in and send to me - it gets most things covered and I always contact them later to finalise things anyway! Hope that helps:

The one I posted isn't my contract that is just the order form that I use when I take an order. I have that written on the bottom, that the customer understands that there is a contract with all the ordering, renting and cancellation policies. If you need that I can send that too.


Edited to add:

My contract is a 6 page contract with very specific details about cancellation, rentals, delivery, pickups and ordering. If you would like a copy let me know I can send it with the Order Form.
