Actually, if I were a 20 year old twin, I would want my own small cake with my own interests on it.

I had a request for a dual birthday cake for a girl and her cousin. I made a pair of sandals so they technically had their own cakes but it went together. The customer said they really liked it.

I have 20 year old twins, and I would recommend two cakes reflecting the individuality of each. On their birthday, the celebration of their birth is important to them, not the fact that they were born a twin.

A good idea would maybe be something that would go together.

Make the cakes into the shapes of jigsaw pieces so that they could fit together (technically) and then customize it to each of their interests (doesn't need to be that exactly but something like that would probably work). Something like that could make it so that the cakes symbolize "twins" or "sisters" but still give you the chance to individualize each cake.
