I remember reading on here that some of you send a survey to your customer after delivery or pick up. I was hoping to do this and would love if I could see what you send. Or if you could tell me some of the questions you ask...

I send one out. I send it with my receipt for the cake.
I just ask about professionalism, taste, price, and how well did I capture their "dream" cake and how did they find out about me.
I don't send them to everyone, because I do offer a "perk" for them sending it back.
i had some postcards made on vistaprint, with the questions and my return address printed on them. i just add a stamp and give it to the customer when they pick up a cake. the majority come back, but i think i also need to add a perk to get them all returned. I tried to make it as easy as possible, all they have to do is check the boxes and drop it in the mail. how hard is that. too much for some!!
I also send out a questionnaire after ea delivery. Just general ?? like taste, price, size with a box to tick bad acceptable good or excellent. I also have a scetion to add some comments. I dont offer any "perks" but I get back 90%

I also have self-addressed postcards made at Vista Print. You can get 100 for free (just pay shipping). I just got a "free" rubber stamp, and 100 more postcards - and it cost me under $10 for the shipping.
I send this out in an email shortly after the wedding. I get quite a few back and it gives me text for my testimonials page on my site.
