I don't know. If it were me, I guess I would make a "frame" for the picture out of fondant or chocolate...then just write beneath it. I always like hearts too if its supposed to be a sweet rather than funny cake. either fondant or chocolate hearts--I usually angle them up off of the cake a little rather than laying flat. The picture will be your focal point and so I guess everything will revolve around that.
You could always go to hallmark.com or another free e-cards site for ideas under anniversaries (if you want to say something other than "happy 40th anniversary"- I do that a lot! Good luck!
I put my message on a scroll with roses at the corners for anniversary cakes. theres a few pictures in the gallerys. you could put their picture on the scroll too.
