I did a shoe for my daughter's 21st birthday. It's in my photos. I took a shoe out of her closet and drew the pattern for it. Then I discovered "socake"'s shoe, and she has written an entire tutorial on how to make a shoe from fondant. It is awesome. Maybe you can PM her for it. The main thing I couldn't figure out was how to shape the toe part -- duh! Socake used a rolled up parchment cone! Worked perfectly.

And I just globbed a bunch of fondant on a dowel and shaped it to make the heel.
