I think it's Narcotics Anonomous. And I think I'd just do a cake in happy colors, something springy that says congratualtions, so and so....Or ask your client, but you might want to use the phrase one day at a time etc...

cake that looks like a stack of calendar pages..

with each day crossed out

and the anniversary one circled in red.

I would suggest doing a cake that has the NA logo on it and then do a banner that says congratulations or happy 1st anniversary.
