Shirt Cake?

I've seen so many nice men's shirt cakes in the Father's Day section, and was wondering if anyone can give me any tips if I try my 2nd fondant cake for my husbands 35th this weekend. Like, do you sculpt the shirt or just use a single layer cake from a rectangle pan?

How do you get the collar looking right....someone mentioned paper under it?

ANy help would be appreciated...the hub is a fan of Ace of Cakes and would get a kick out of me trying something unique. (unique to me, obviously not to you gals).
