Have you considered making a sheet cake with the different tools that a physical therapist uses? Like those blue mats, balls, even bars, pool, trampoline, resistance ropes or weights.

Maybe you could incorperate a medical clip board/chart or medical bag so that it is not too much like a gym...

Hope you are able to make something great!

What about a variation of Debbie Browns massage cake? Look at http://www.debbiebrownscakes.co.uk/cakes_for_men/pages/massage_jpg_jpg.htm

Your physical therapist should wear more clothes! icon_lol.gif You also can make the patient the other way round if you don't want a massage. Then the patient could do some ab crunches.

Another idea:
A bone (femur), a vertebra or the whole spine.
