What Type Of Forms Do You All Use For Your Business?

I'm just starting out and through word of mouth at work, I have three girls wanting me to make wedding cakes for them! I know I need some type of form for them to fill out as far as date, time, etc. including prices and such. I was also thinking about putting a packet together for brides that would include a cutting guide, tips to freeze their top tier, and maybe a few other items.
I was wondering what type of things you all put together for brides and where you find this information to copy for them. Any help or direction would be great! Thanks in advance!

I would do a search of the forums for "Contract" "Wedding contract" I know some people on the boards have posted the exact contract they use and you may be able to modify it.

I have one I can e-mail to you if you'd like. PM me your address if you'd like it, it is a verson of Earlenes, but I tidied it up a bit and revamped it to work for me.
I use all kinds of forms, I just love making them up. I have a customer call sheet for wedding inquiries, this is basically name, phone, wedding date, email address, approx guest count, date of consultation, and notes.
I also have a wedding cake worksheet that I use during the consultation. It has space for the bride/groom to put all of their information, wedding contacts, etc, then space for me to write cake details, and on the back is a summary area to work up a price estimate.
Then, I have the formal contract which I type up while they are doing the tasting, I'm out of the way and they can discuss what they like/dislike.
For non-wedding orders, I have a simple order form that I made up. I keep them all in a folder sorted by date.
Those are all good ideas...... I need to go create some! I just have my excel spreadsheet to help me with pricing, but having the forms described is a great idea.
I was thinking also for each type of cake that you do, to make a menu of sorts. Has the different design types (they have this in Wilton, but I would restrict it to the more common ones I do) in basic shapes created in word with 3D cirlces or something. Then include what kind of extras they can add on (filling, flavors what not) and have check boxes the bride can check as to what they might be interested in.
Earlene Moore has all that information (including sampe order forms) on her website... I used her information and altered it to fit my business.
