It's an optical illusion. A tube is used as support and the stems are placed around it. It couold even be styro as long as it is completely covered w/the stems wich are fondant ropes.

There was a thread on how that was done but I forget the exact wording for the title of it. I just remember there was a similar picture and the artist did a lengthy explanation for it.
