How much would you charge for a double layer 13x9 cake covered in buttercream with a rice krispie, fondant covered book (hand painted) similar to the photo below?[/img]

Your price will depend on your cost, what do you expect to spend on ingredients for the cake, how long will it take you to prep/bake/decorate/clean for this order, and how much overhead do you have (insurance, license fees, etc.)?

Also, the picture you posted is copyrighted, so if you haven't done so already you would need to get permission from the copyright owner before you can copy it on a cake.
