Please Help!!

I have been asked to make this cake and i have no clue how to do the middle blocks in between each tier .... what material do i use?  i have only done three tier cakes .... will this be difficult ? i do know that what ever that is it is covered in the plastic rhinestones- but what is it and how do i do this?
