According to joyofbaking.com add 7.5 grams baking powder and 3.5 grams salt per 140 grams of all-purpose flour.

To make one cup of self-rising flour:

* 1 cup of all-purpose flour
* 1 1/2 teaspoons of baking powder
* 1/2 teaspoon of salt

Sift together.

Note: Unless a recipe calls for self-rising flour and yeast specifically, do not use it in yeast dough recipes.

Found this on several sites and the recipe was identical.
