Hi, I made one a couple of weeks ago (in my photos), and I stacked cakes to just a bit bigger than I needed, and then carved it, keeping a real boot next to it all the time for comparison. To cover it, I took rough patterns from a real boot, cut them from rolled fondant icing, and then 'built' the layers, ie tongue first, then sides and toe-cap, etc.
I didn't find it too difficult, but I think you really need to get the shape and proportions right to begin with, or you will never get it to look right!!
