That's all hand-piped royal icing.

It looks hand done to me. Some of the lines aren't perfectly straight, but such a beautiful cake. I Googled Cake Lace Mat and scrolled the images, there were a couple that look close to this. Unfortunately my phone won't let me link them to the post.

You could press lace into the cake and use the impression as a guide.
