You can use an actual cupcake or rkt.  If you go to youtube and look up "how to cake it", she just did a cake with  a cupcake on top in the last week or so.  She showed how she made the topper in the video.  If I remember right, she used a cupcake.

jinkies you are right.  It was Yolanda Gamp and she used a cupcake covered in fondant for the bottom and used fondant for the look of the icing on top.
