OK, I'll answer #1 for ya.
Some questions that I ask clients are:
1. When do you need this cake for (date/time) and where is the place of delivery?
2. How many servings do you need?
3. Do you have a particular design in mind; do you have pictures, etc?
4. What is your budget?
5. How did you find me ( I keep track of my referrals)?
Once I get the basics, I can come up with a price quote and if they decide to go further and pay their deposit, we move on to cake flavors and minor things like that.

Hi Tinychef, I'm in Australia too, as K8memphis said the answers to your questions are quite complex. We regularly take Year 10,11,12 students in our kitchen for work placement and I have helped out with information for both Hospitality and Business Study students doing their HSC. Flavor, date and time, and location where needed. Will they pick up or must it be delivered? What else is already on my calendar for that date.
