I am planning on making a cake similar to this for my Grandmas birthday. I can not tell if the ribbon they use is fondant or real. How would you use real ribbon on a fondant cake? Also would you have to take it off before you cute the cake? Also the cake shows little pearl candies all on it. How should i put the pearls on so they stay?

yes, that ribbon appears to be real fabric ribbon.  Wash it, then cut to size and glue the ends together with a bit of RI.

Pearls - mark the swags carefully and very carefully apply each pearl with a tiny dot of RI.

I would just use fondant to make the ribbon. That way, you wouldn't have to worry about taking it off (or explaining to someone how to take it off) and you can easily pipe the tiny dots over it without worrying if they'll fall off.
