Help With Idea For Retirement Cake (Medical Reasons)

So my stepbrother has just been diagnosed with ALS also known as Lou Gehrig disease so he is having to take a medical retirement, in his 40's with a year to a year and a half to live.  I have been given the honor of baking the cake for his retirement party.  Any ideas for decorating, it isn't like he is going to be off fishing, or playing golf?  I think a clock might not be in very good taste.

My prayers will be with your step brother  and for the family. It is a very hard thing to go through watching a loved one having to suffer!!! I really don't have any ideas but I am sure others here will. God bless you and yours!!
