By using an embosser?

That looks to me like all hand piped using a very small piping tip. Maybe a Wilton tip 2 or 3. Looks like royal icing was used.

Yep....a #2 tip, a comfortable chair and some softened butter cream. I only use royal for cookies. I add more liquid to a little bowl of buttercream and make it super smooth so it flows out without any airbubbles or breakage.
