Looking at it it looks like maybe some gumpaste cutouts and piped royal icing for the white decorating on black, but I feel like maybe I'm missing something? Wow, what a cake. I think you're right about the white cutouts and royal icing on black fondant - that plus plenty of patience and a steady hand!
