I'm making a wedding cake for a friend and I'm not a pro. She changed the design on me recently and her theme is black and white. So I'm thinking of doing a design that's somewhat like this:

It would be greatly appreciated if someone could tell me how this sort of lace design is achieved. Do I need a machine? Can I just do it by hand?

My guess is it is a stencil.

Yep it's a stencil. You can get them at global sugar art or even at your local crft store.

You can do it in butter cream or royal.

Another option would be cutters or the circuit.
