I found this cake on CC

How do the pumpkins hold up the top tier? What are they made out of?

They could be Wilton globe pillars.

In the comments she said that she used the Wilton globes and pillars. If you are not able to find it you can use styrofoams balls covered with the fondant and toothpicks or straws to insert them on the cake and in the upper part use a glue gun to attach the 2nd tier to the top of the pumpkins. HTH

The pumpkins aren't supporting the top tier, the hidden pillars are.

If you aren't using the globes, whatever you decide to use would still need to slip over the hidden pillars for support.
