It's a bit hard to see exactly but it could be gel or just b'cream colored via airbrush.

Definitely airbrushed, but did they airbrush first then with a stencil then piped after? Check out this one..

It looks to me like it was all airbrushed with the help of stenciling, just the dots on the crown were piped then airbrushed, and the rattle looks like it has a piped ribbon.
