If you google "respiratory care week" there are some sites that have some cool pictures that you could put on the cake (FBCT or freehand) One site relating to Positive Promotions had a simple one that would look nice.

If you do anything with fondant, a stethoscope would be cute.

Some other ideas to throw out - a nasal cannula or oxygen mask/tubing, the medical caduceus with an R and a T on either side.

I am a nurse in my other life...

How about a peak flow meter, or a person blowing into a peak flow meter.

Or....I can't remember what the machine is called....but it's the one where the patient blows into it as hard and as long as he possibly can. You could use this idea and have a patient blowing into the machine with his eyes bulging out from blowing so hard, and from the lack of oxygen.

The first thing popped into my head is, you take my breath away. If it's for an appreciation thing you could use that maybe.
