the trick is the pan used to make the belly.

depending upon size needed and how preggers you want it to be, it can be:

for a round belly -- that "what you got there -- a baby elephant" effect!
soccerball --- big!!!
half of sports ball -- small

for a more natural oval, pear shape "incoming -- she' dropped!"
football pan, carved to remove one of the pointed ends -- big
easter egg pan -- no carving -- big
mini-easter egg pan -- small
mini-ballon pan (rare pan) -- small

these pre-shaped pans make it real easy.
and w/ the mini-egg or balloon, could do multiple ones each gets her own cake.

then match corresponding pans for the breasts.

so if doing the football (incoming!" well then two mini wonder molds will look like she's about to burst if she doesn't nurse that baby upon delivery.

or if could make the small half round balls (mini half ball pan) or could cut up cupcakes or even just make little balls of fondant for breasts

assemble the belly and breasts and ice w/ BC to smooth out/blend the shapes.

then drape w/ piece of fondant for the dress and arrange pleats as desired. some prefer to drape belly w/ once piece and breast with another and then hide the joint w/ a belt and bow. really up to you how you want the dress to look...but still simply a piece of fondant -- draped, trimmed and pleated and gussied out w/ feminine fru-fru.

and here's a whole bunch of pictures to inspire:
(note the one that just used a round tier cake for the belly -- wonder if she was doing it for someone have quints or more!?!?!)
