Woman's 50Th Bday Also Celebrating Breast Reduction - Ideas?

I've been asked to make a cake for a woman's 50th and they'd also like to celebrate that she's just had a breast reduction... my initial thought - before they mentioned the breast reduction... was to do the "bra cake" - let's show her some support thing... but now I'm not so sure that would be the best choice. Any ideas on how I could spin it a bit differently or on a totally different cake concept?

stick w/ the bra


she just needs a new smaller size (gift for b-day)


and then

also make a play on fact that 50 is the new 40 (see -- even that's been downsized!

so a DOWNSIZED Birthday

and if want to be a bit vicious -- make a bday cake just for here that is just two cupcakes stuck together --- really itty bitty bra!
