looks like a 6 petaled blossom cutter dried in a rounded form (like an egg carton Leah suggested). I personally would use fondant on the cake and royal icing to glue the flowers on. There are three little dots in the middle of the flowers that can be piped in royal icing.
AYou might consider taking the beginner's Wilton flower course at your local craft store, they cover things like this.
