I thought previously someone said that there was the 3d pattern put on the cake first, and then the whole thing was covered in fondant, and pushed in around the pattern beneath it so it gave it the dimensions. Jay Qualls did it if you want to google it some.

I would apply fondant or modeling chocolate in the design you want directly to a crumb coated cake and cover it all with fondant. Use a ball tool to gently stretch fondant into all the crevices. I would then airbrush and use a sponge with some vodka and wipe over the raised pieces to give the worn effect.

Hey! I did a cake last month just like this. I rolled out some thick fondant and cut my pattern out of that; then I let those dry. Then I stuck them to the sides of my cake, rolled out the fondant to cover it all and pressed it in around my designs. I used some tools after it was on there good to help define the designs. I'm sure it would be easier if you had molds or with modeling chocolate, but what I did worked out great too.
I've got pictures of this cake in my gallery. It's the "tooled leather cake" I'd attach it for you, but I don't know how to do that yet! LOL

You can look at it big here.

I agree with what everyone says about the designs being stuck to the sides and then covered with fondant and airbrushed. I saw this done in one of the Amazing Wedding Cakes episodes.
