You can get a chocolate mold for the bottle here at Country Kitchen Sweetart.

Just use some melted yellow chocolate wafers to paint in the yellow part and then fill the rest with white chocolate.

As for the pacifier, I would just cut a circle out of fondant, shape an oval piece for the part that the child sucks and attach them both to a handle rolled out of a long piece of fondant. There's a picture of both on the diaper bag cake in my photos.
